Have to go and collect a form before you can get an appointment - so have to take a day off work to collect a form as they are not open on weekends. Form not available to download as that would be too sensible.

The receptionist at Maple Access is disgusting! I can not believe how I have just been spoken to and it's not the first time this has happened. She got my date of birth wrong she kept saying the 25th April when my birthday is 26th of April. After having to tell her my date of birth four times she told me "do you want to make your mind up!" So rude! She then huffed and puffed down the phone at me when I couldn't make the appointment and when I ask if I could come another day because I'm at work. There is no need to talk or treat people like this. Learn to have some respect and manners when talking to people. You are talking to people are unwell and need help.

A total waste of time , and i did more harm to myself by going out in the rain and cold to see a doctor ho i was expected to give me some medical advise and actually show interest in my condition , i was treated with the most speed , the gp did not know how to kick me out faster , i whent there to get answears , receive a medical treatment , i received nothing just told go home and take paracetamol and ibuprofen ....
