Story

Glendale Surgery is committed to providing high quality and readily available health care to the patients of this practice. We aim to offer continuity of care with the doctor of your choice as much as possible and see this as the key to developing a good doctor/patient relationship.

We welcome new patients to the practice. You will need to complete a GMS1 and a Patient Questionnaire for each new patient which can be downloaded and posted or handed to reception. When you bring the forms to the practice, you will need to bring proof of your identity.
